Allegheny County Department of Human Services, Area Agency on Aging (AAA)

Brief Description
The AAA is one of 52 Area Agencies on Aging within the state of PA. Locally, it is under the umbrella of the Department of Human Services (DHS). The mission of Allegheny County Department of Human Services is to plan, organize, coordinate, convene, advocate, programs and funds where possible, services to elderly and needy residents of Allegheny County.

Role in EXPORT
AAA Administrator will serve on the community advisory board, participating in planning meetings, and providing advice and consent regarding the participation of African American in research activities. AAA will be involved in the aging pilot study submitted by Lemington Eldercare Services that involves training lay aging advisors in the churches and beauty salon and getting input from seniors on their motivation for accessing services.
Centers for Healthy Hearts and Souls (CHHS)

Brief Description
Collaborative includes twenty churches with an estimated 35,000 people. The Center for Healthy Hearts and Souls presently operates community based smoking cessation and prevention programs, fitness and nutrition programs for men and women, diabetes support groups for adults and children, Youth Health Corps Educators, Health Ministry health promotion and disease prevention activities.

Role in EXPORT
The Executive Director of CHHS will serve on the community advisory board, and the CHHS will provide technical assistance to the Research cores and Pilot core on engaging the faith-based community. CHHS will also help identify consumers in the faith community for lay health advisor training and be involved in the training on coalition building and other trainings offered to partners. They will also be involved in all Community Outreach and Information Dissemination Core activities, including the communication campaign and information dissemination.
Center for Pediatric Research

Brief Description
Consultant

Role in EXPORT
The head of the Health Promotion and Disease Prevention Section at the Center for Pediatric Research (CPR) will serve as consultant. She will meet (via conference calls) with the Principal Investigators, staff and community members to plan and facilitate two trainings specific to their needs on building effective health promotion coalitions and how to sustain them.
Commonwealth of Pennsylvania Department Of Health

Brief Description
Secretary of Health for the state of Pennsylvania.

Role in EXPORT
The Secretary of Health will serve as an advisor to CMH for Project EXPORT.
Coordinated Care Network(CCN)

Brief Description
Faith-based collaborative that seeks to provide non-fragmented, whole-person preventive care across medical, social and behavioral health disciplines for at-risk and uninsured populations in urban Pittsburgh. Formed by 12 safety-net providers in 1996, CCN offers more than 194 different programs at 79 operating sites, where approximately 57,000 patients are cared for annually.

Role in EXPORT
The CEO will serve on the community advisory board, participating in planning meetings and providing advice regarding the partnership of African Americans in research activities. CCN will help identify consumers in their network for lay health advisor training and be involved in the training on coalition building and other trainings offered to partners. They will also be involved in all Community Outreach and Information Dissemination Core activities, including the communication campaign and information dissemination.
Jackson State University

Brief Description
Fourth largest educational institution is Mississippi, located in the states capital and largest metropolitan areas, is the seventh largest of the 117 Historically Black Colleges and Universities. Jackson State offers 40 bachelor's degree programs, 37 master's degree programs, 9 specialist degree programs, and 10 doctoral programs. It is also the lead institution involved in the Jackson Heart Study, the largest prospective epidemiological study of cardiovascular disease in African American ever undertaken.

Role in EXPORT
Three-four students and one-two faculty from Jackson State University will come to Pittsburgh every year to participate in a weeklong Summer Research Institute for Elimination of Racial and Ethnic Health Disparities. This will provide an opportunity to share ideas and specifically to develop joint educational modules on minority health and health disparities.
Kingsley Association

Brief Description
Based out of Pittsburgh Pennsylvania is an organization that hosts health and wellness activities serving residents of Pittsburgh's East End communities. These efforts include a stroke survivors' group, health education workshops for community residents, and youth focused health and wellness activities, along with support for minority HIV/AIDS Educational Outreach.

Role in EXPORT
By working with the Community Outreach Core on strategies to promote health and prevent disease among African Americans in the East End of Pittsburgh. Kingsley Association will identify consumers through their network for lay advisor training and be involved in training for coalition building, participatory research. They will also be involved in all Community Outreach and Information Dissemination Core activities.
Lemington Center

Brief Description
Continues to serve the elderly community and admits residents regardless of age, race, religion sex national origin or financial status. Serving the primarily the eastern portion of Allegheny county, out patient population consist of 115 medically complex individuals with diagnosis ranging from cardiovascular disease, cancer, hypertension, AIDS, and various complications due to drug and alcohol abuse.

Serving on the community advisory board, participating in planning meetings, and providing advice and consent regarding the participation of African Americans in research activities. They will also be involved in all Community Outreach and Information Dissemination Core activities, including the communication campaign and information dissemination.
Maurice Falk Medical Fund

Brief Description
The Falk Medical Fund is a grant-making foundation incorporated in 1960 as an outgrowth of the Maurice and Laura Falk Foundation, which was established in 1929. For more than 30 years, the fund's focus has been social policy related to mental health,
community health, civil rights and minorities issues.

Role in EXPORT
Serving on the community advisory board and helping to identify sustainable funding opportunities for EXPORT. The President of Maurice Falk will also serve as the liaison between the EXPORT Center and the Pittsburgh Funders Forum (local foundations interested in health disparities).
Morehouse School of Medicine
Brief Description
Consultant

Role in EXPORT
To serve as a member of the Center's External Advisory Council and as a consultant to the Immunization and Pneumonia Disparities Research Core.
Pittsburgh Pastoral Institute

Brief Description
A community based licensed outpatient psychiatric clinic.

Role in EXPORT
Serving on the community advisory board, participating in planning meetings, and providing advice and consent regarding the participation of African Americans in research activities. The Pastoral Institute will also help identify consumers in their network for lay health advisor training and be involved in the training on coalition building and other trainings offered to partners.
Pittsburgh Theological Seminary

Brief Description
Associated with over 300 churches, community based agencies, and other educational institutions to work and collaborate with organization. The Pittsburgh Theological Center and the affiliated religious congregation serve over 24,000 residents in the Greater Pittsburgh area. The aim of this coalition is the reduction of health disparities between predominately poor and/or African American communities and the Pittsburgh region in general.

Role in EXPORT
Serving on the community advisory board, participation in planning meetings, and providing support for research at the Center for Minority Health. In addition, The Pittsburgh Theological Center agrees to collaborate with the Freedom Mentoring and Leadership Development Honor Society to develop and implement the pilot Health and Science Scholar Project.
Primary Health Care Services, Inc.

Brief Description
Primary Health Care Services, Inc. is comprised of Alma Illery Medical Center, its headquarters, and six satellites: Lincoln Park, Manchester, McKeesport, Rankin, Sheraden, and West End. - all in medically underserved areas. The community based health centers provide a range of comprehensive healthcare services, including general medicine, physical and gynecological exams, premarital physicals, laboratory tests, pediatrics, immunizations, dental services, psychiatry and psychosocial services, health education, podiatry and minor surgery.

Primary Health Care Services, Inc. will also help identify consumers in their network for lay health advisor training and be involved in the training on coalition building and other trainings offered to partners. RAND

Brief Description
RAND is a nonprofit institution that helps improve policy and decision-making through research and analysis. RAND's area of expertise: child policy, civil and criminal justice, education, environment and energy, health, international policy, labor markets, methodology, national security, population and regional studies, science and technology, social welfare and transportation.

Role in EXPORT
By developing a formal relationship with the Export project and enabling RAND to provide important support and assistance in the ongoing development and growth of this important initiative. RAND has worked with the EXPORT partners to develop the strategic plan for this initiative and will continue to provide technical assistance related to the implementation of that plan and provide support and assistance in the ongoing development and growth of EXPORT.
United Way of Allegheny County

Brief Description
United Way of Allegheny County is a nonprofit organization which enhances
the Pittsburgh region's quality of life by nurturing children and youth, supporting older people, promoting health and wellness and increasing Self-sufficiency. In addition to mobilizing all segments of the community to raise funds for health and human service programs, United Way promotes volunteerism through its annual Day of Caring and Good Neighbors volunteer center.

Role in EXPORT
By serving on the community advisory board, participating in the planning process and supporting the implementation plan. They will also be involved in all Community Outreach and Information Dissemination Core activities, including the communication campaign and information dissemination.

Community and Governmental Relations

Brief Description
A multi-disciplinary initiative funded by the U.S. Department of Housing and urban Development to mobilize university resources to respond to pressing community issues.

University of Pittsburgh
Freedom Honor Society


Brief Description
As the first minority, student honor society in the history of the University of Pittsburgh, Freedom Mentoring and Leadership Development Honor Society was established to aid in the retention of historically underrepresented students. The mission is to produce highly trained and effective student mentors, who in turn, will help significantly improve the academic, cultural, social and personal development of underrepresented students.

Role in EXPORT
A representative will serve on the community advisory board, participating in planning meetings, and providing advice regarding the participation of African Americans in research activities. Students will help identify other students in their network for lay health advisor training.
University of Pittsburgh
Health Policy Institute


Brief Description
Is an independent, community supported and non-partisan source of educational programs and services for elected officials-local, state and federal-as well as civic, non-profit organizations, business and foundation leaders.

Role in EXPORT
Serving on the community advisory board, participating in planning meetings, and providing advice regarding the participation of African Americans in research activities. EXPORT will be an extension of some of the past co-sponsored activities, such as lecture series and special forums for PA legislators as a mechanism for the dissemination of information related to EXPORT.

University of Pittsburgh
Institute of Politics

Brief Description
An independent, community supported and non-partisan source of educational programs and services for elected officials - local, state and federal - as well as civic, non-profit organizations, business and foundation leaders. Operating under the auspices of the University of Pittsburgh, the Institute creates and encourages opportunities for faculty, students and the broader community to join these leaders in the examination and discussion of difficult public policy questions confronting the Western Pennsylvania region.

Serving on the community advisory board, participating in planning meetings, and providing advice regarding the participation of African Americans in research activities. The Institute will also work in partnership with the CMH to establish a Legislative Health Disparities Working Group in Harrisburg.

School of Dental Medicine


Brief Description
Provides pre-doctoral and residency education to practitioner in dental medicine. It serves the tri-partite mission of the University of Service, education, and research. The School serves the needs of all ethnic populations and is strongly committed to enhancing its position in minority health.

School of Dental Medicine will also be involved in cultural competency activities of the Minority Health Disparities and Education Core.
University of Pittsburgh
School of Nursing

Brief Description
Offers educational programs hat anticipate and reflect health care needs locally, nationally and internationally. It prepares graduates to function effectively in multifaceted roles in a variety of settings to promote the health and well-being of people.

Role in EXPORT
To participate as a consultant to the Minority Health and Health Disparities Education Core working in collaborate with the core director, Dr. Jeannette South-Paul. School of Dental Medicine will also be involved in cultural competency activities of the Minority Health Disparities and Education Core.
Urban League of Pittsburgh

Brief Description
The Urban League of Pittsburgh has an 83-year history of initiating programs that respond to he needs of African American community. Those programs and services have focused on the most basic needs of employment, fair housing, education, family support and racial justice. The Urban League of Pittsburgh has been a leader in working with government initiatives in these and many other areas, including healthcare. In the 1960's the Urban League advocated for low income and minority members to be represented on Pittsburgh's Comprehensive Health Planning Association (CHPA) and on CHPA's successor organization.During the same time period, Pittsburgh was the first Urban League affiliate in the country to establish a separate health department of its own staffed by a full time health professional.

Western Pennsylvania Caring Foundation, Division of Community Health

Brief Description
A non-profit affiliate of Highmark, The Caring Foundation promotes health and well-being in Pennsylvania and elsewhere by providing services in connection with, and sponsoring programs related to children, elderly and economically disadvantaged persons in the community. The Division of Community Health is dedicated to improving the health of underserved individuals, special needs populations and at-risk communities. The Division will maintain its commitment to building healthy communities.

Role in EXPORT
Serving on the community advisory board, participating in planning meetings, and providing advice regarding the participation of African Americans in research activities.The Division of Community Health will help identify consumers for lay health advisor training and participate in the training on coalition building. They will also be involved in all Community Outreach and Information Dissemination Core activities, including the communication campaign and information dissemination.

Youth Enrichment Services, Inc

Brief Description
A non-profit 501 © corporation, was formed to specially design and implement educational programs for minority children living in public housing and economically disadvantages communities.

Role in EXPORT
Serving on the community advisory board, participating in planning meetings, and providing advice and consent regarding the participation of African Americans in research activities. They will also help identify consumers in their network for lay health advisor training and be involved in the training on coalition building and other trainings offered to partners. They will also be involved in all Community Outreach and Information Dissemination Core activities, including the communication campaign and information dissemination. They will also participate in a pilot study related to obesity in young girls, and work closely with the Obesity and Nutrition Research Core.
YWCA

Brief Description
Founded in 1867, the YWCA of Greater Pittsburgh has been pursuing it mission of creating opportunities for women's growth, leadership and power. Central to this mission is the imperative to eliminating racism, which is a major obstacle to attaining the vision of "peace, justice, freedom, and dignity for all people".
